GRANT APPLICATION CAN BE FOUND ON OUR WEBSITE. FIVE COPIES OF THE GRANT REQUEST APPLICATION MUST BE SUBMITTED BY MAY 1 OR NOVEMBER 1. AN EXPLANATION OF THE PROJECT FOR WHICH THE GRANT IS BEING REQUESTED SHOULD BE PROVIDED ALONG WITH A SUMMARIZED FINANCIAL STATEMENT OF THE AGENCY AND PROJECT. PROOF OF NON-PROFIT EXEMPT STATUS MUST BE PROVIDED. A LIST OF CURRENT AGENCY BOARD MEMBERS, DIRECTORS, OR OFFICERS MUST BE INCLUDED WITH THE GRANT REQUEST.
