The mission of the Colorado Health Foundation is to improve the health of Coloradans. It operates on three cornerstones: serving Coloradans with less power, privilege, and income, especially prioritizing those of color; being informed by the community it aims to serve; and focusing all efforts towards creating health equity.
This funding opportunity aims at improving the capacity of clinics in Colorado to provide high-quality, team-based primary care specifically tailored to the needs, preferences, and values of Coloradans of color. It offers capital grants of up to $400,000 to safety net practices that are either implementing or have already implemented a team-based care model. To qualify, clinics must demonstrate that the foundational elements of team-based care are present or in progress. The specific projects that can be funded include facility acquisition or improvements, purchasing medical and office equipment, supporting telemedicine access and patient education in telemedicine use, and expenses related to health information technology systems. Eligible organizations for this funding include federally qualified health centers, rural health clinics, community safety net clinics, school-based health centers, and community mental health centers offering integrated primary care services.
