Overview

The grant program aims to promote an equitable and cost-effective health and behavioral health care delivery system. It focuses on advocating for successful health care reform, building the capacity of New York City’s health sectors, reducing health disparities in low-income neighborhoods, and fostering the independence of people with mental illness and substance use histories. The program supports projects targeting low-income individuals and communities and focuses on making communities accessible and inclusive, ensuring high-quality services for people with special needs, and building the capacity of nonprofits serving these populations through workforce development, financial and management systems enhancement, and utilization of new financing mechanisms.

The strategies for Health and Behavioral Health as well as Older Adult and People with Disabilities prioritize projects that provide solutions which are comprehensive across the sector, systemic, and involve collaboration among multiple agencies. Additionally, these strategies often seek to collaborate with other program areas within the Trust to maximize the effectiveness of their grants.
